Medicare as it exists is a big help, but far from ideal. Otherwise, why do so many of us need additional coverage, like Medicare Supplemental or Medicare Advantage plans? And a lot of these still leave a big out-of-pocket.

And then, how to finance it?
First, a merciless crackdown on fraud.
Payroll tax?
Income tax?
Higher tax on obscene incomes?
